Application Process Overview

1. Once you have passed your CNA exam (both written and skills) you may complete the Certified Nurse Aide Recruitment Program Application and submit the necessary documents. Endorsement applicants do not qualify for the program.
   Note: If you do not know your registration number to log into the form, you may go online to the verifications page to obtain your number.

Required Documentation and Information

Prior to beginning the application please ensure you have all the correct documents. The documents will be required to be uploaded.

• If you answered "Yes" to any of the questions in the Personal Background History, an explanation and any relevant documentation as requested at the end of that section must be submitted with the application. If you cannot send a copy of the criminal judgment, please include that in your explanation.

Application Processing Timeline

Completed applications are processed within 3-5 business days. You are able to check the status of your applications online using the Application Status.
